Report a Student Medical Condition

Your student health and safety is important to us! Please report any medical conditions your student has or any medication your student needs using our online Health Form. For students with asthma, allergies, diabetes or needing medication on site, we will need your child's doctor to complete a school action plan. You can find the link to the action plans below.

For students that are new to OUSD, such as incoming TK or Kindergarten students, we MUST have updated immunization records before students are allowed to begin. Immunizations can be turned in to our Main Office.
